Community Medical Alliance Clinical Group (CMACG) is one of the first Medicaid managed care practices in the nation to provide community based primary care to adults with severe disabilities such as mental retardation, severe mental
illness and HIV/AIDS. Due to a unique relationship with our affiliate, Neighborhood Health Plan, our nurse practitioners provide AND authorize care and services, ensuring that clinical needs drive all care. The Nurse
Practitioner provides direct care in the patients' homes, care management and coordination across the continuum of care, working in
collaboration with the PCPs and providers.
Position Responsibilities:
Provides primary care, care management and overall coordination of the plan of care to a specified panel of medically complex children in collaboration with the primary care physician. Works with Neighborhood Health Plan, the Massachusetts
Division of Medical Assistance and the Mass Department of Social Services to develop and define the program model and
operations.
In this role, you must possess a demonstrated competency, experience and comfort with the care of technology dependent children (ventilator dependent, tracheostomy, gastrostomy, enteral feeding, etc.) Proven skills and knowledge base necessary for independent clinical decision making. In addition, you must be highly organized, able to function as part of a team, and, communicate effectively orally and in writing. You must also possess strong interpersonal, customer relations and effective teaching skills.
Education & Experience:
Master's degree in Nursing, ANA certification as a pediatric or family nurse practitioner current Mass License as an RN/PNP or RN/FNP required. Minimum of 3 years relevant clinical experience.
