Directly responsible for a group of participants in a population to identify and recommend a plan of care, which ensures quality care, while providing cost saving initiatives. Will be responsible for all of the following: Evaluating physical & psychosocial factors in a holistic manner; Establishing a comprehensive plan of action based on participant needs, participant/caregiver preferences and existing or proposed treatment options; Identifying needs and coordinating resources based on the participant's need's and preferences; Continually assessing the plan of care for effectiveness in meeting goals and tracking outcomes. 

***MUST be bilingual (English and Spanish) and a Professional RN to be considered*** We are targeting a nurse with 4-7 years of professional experience.
